Please go to “Forms” and complete all forms pertaining to your needs. All clients need to complete the Telehealth if it was a telephone or ZOOM evaluation/assessment, a questionnaire, an information consent, and a contract for your specific evaluation. FV stands for Family Violence, AM stands for Anger Management. To complete, click on the form and if the Acrobat pops up you can choose the text box application and type in the information. Save to your computer and then email the forms to laurie.ainetwork@gmail.com.

How to get started:
1 .Download ZOOM app on your computer, laptop, or cell phone (ZOOM cloud meetings)
2. Pay for your classes with these options:
Cash App to $ainetwork (lowercase ‘i’ in the cashtag, the name will show up as David Clark).
Click the Pay Now link at the top of this page.
Call the office to process your card over the phone using any of the numbers on the HOME page.
Drop off cash, please schedule an appointment with Dr. Clark at 770-713-8580.
ZELLE the balance to (770) 713-8580.
3. Please text a picture or screenshot of your payment receipt to your facilitator prior to the start of your group. The receipt must contain the date. The name and number to text the receipt is listed as the Contact for each class on the schedule below.
4. JOIN a meeting and enter your meeting ID and passcode without dashes or spaces. Your class has the same meeting ID and passcode each week.

Do you need an alcohol and drug evaluation for probation, an attorney, the Department of Transportation, for your job’s Employee Assistance Program, or for any other reason? At Atlanta Intervention Network, we provide state-approved evaluations and treatment for ASAM Level 1 drug and alcohol use. We also provide educational needs, for individuals who have charges related to DUI, drug use or possession, positive drug screens, and CDL’s. Clinical Evaluations on the State of Georgia’s DHR Registry for individuals seeking an evaluation for driver’s license reinstatement as well.
If you need an evaluation, we are here to help. We have 6 locations throughout the metro Atlanta area and we are available 7 days a week to meet your needs.
