Before your scheduled evaluation, please complete the following three steps:

1. Schedule your appointment.

2. Submit your payment.

3. Complete the Evaluation Intake Form located on this website (scroll below).

The information you provide will help us prepare your file and verify demographic information before the evaluation begins. Most clients complete this form in approximately 5–10 minutes.

Please answer all questions as accurately as possible. If you do not know an answer, provide your best estimate.
