The DOT SAP Process
If you have been reported to the FMCSA Clearinghouse for a DOT drug or alcohol violation, federal regulations require completion of the SAP process before returning to safety-sensitive duties.
All evaluations are conducted in accordance with 49 CFR Part 40.
Step 1: Initial SAP Evaluation
A structured evaluation conducted via secure video platform to assess the DOT violation and determine required recommendations.
Step 2: Education or Treatment (If Required)
Based on federal DOT regulations and clinical standards, education or treatment may be required before moving forward in the process.
Step 3: Follow-Up SAP Evaluation
After completion of the required education or treatment, a follow-up evaluation is conducted to determine compliance and eligibility to proceed.
Step 4: Return-to-Duty Test
Once eligibility is determined, the employer schedules the required Return-to-Duty drug and/or alcohol test.
Step 5: Follow-Up Testing Plan
A federally required follow-up testing plan is established in accordance with DOT regulations.

Regulatory Notice
Completion of the SAP process determines eligibility to proceed with Return-to-Duty testing under federal DOT regulations. Final employment decisions are made solely by the employer in accordance with company policy and applicable federal guidelines.
Who This Applies To
National DOT SAP Services are available to safety-sensitive employees who are required to complete the SAP process following a DOT drug or alcohol violation.
This includes:
• CDL drivers operating under FMCSA regulations
• Owner-operators
• Safety-sensitive transportation employees
• Drivers reported to the FMCSA Clearinghouse
• Drivers who have not yet been reported but require SAP evaluation
• Employees in DOT-regulated positions under FMCSA, FAA, FRA, FTA, PHMSA, or USCG
If you have tested positive, refused testing, or been removed from safety-sensitive duties under DOT regulations, a SAP evaluation is required before returning to duty.
