These documents explain your rights, responsibilities, program requirements, confidentiality, attendance expectations, financial policies, and other information required before beginning treatment. After reviewing the documents, complete the electronic Treatment Acknowledgement Form at the bottom of this page.
You will electronically sign your acknowledgment and acceptance of these forms at the end of the Intake Form and they will be emailed to you.
